Pre-Enrollment Form

Please enter your information as it appears on your driver's license or government issued ID. All enrollment paperwork must be completed before you start class. You are always welcome to enroll in person.

To enroll in the CNA/LNA course, please fill out the form below. You will receive an email confirmaiton with the next steps to finish enrollment.

Tuition and Fees

CPR and First Aid classes are by appointment and should be confirmed via phone at least 48 hours prior. The form below allows a date to be requested. Classes with at least 2 students are preferred. Our AHA CPR course cost is $30. Star Canyon students may receive an additional discount based on exam performance.

Please note that payment of the enrollment fee or tuition online does not mean you have completed enrollment. Enrollment is not complete until you have completed and signed all enrollment forms. You may stop by anytime during our open hours. We are open Mon-Thurs, 9am-8pm.

We are located at 8900 North Central Avenue in Suite 107, Phoenix, AZ 85020.

We are conveniently located along city bus routes on Central Avenue & Dunlap Road.

You may use the button below to upload documents such as a copy of your ID, transcripts/Diploma/GED, immunization records, etc.

Please be sure to include your name.

Once your upload is complete, you will see the words "file path" followed by a code.

You may make payments via the button above. Please use the name used by the student during enrollment in the payment form.

A receipt will be sent to the email and phone number specified. Please save all receipts. If you receive an error during payment, please try the Paypal link here and at the bottom of each page.
